This application is real simple to do, and it only has a couple of really simple steps to complete. Fill in the form below with the requested information. Make sure you enter a valid email address, no email address and the second phase can not be completed. Send the form and in the next couple of days you will receive a verification letter. You MUST reply to this verification letter to be approved, make sure you are not blocking email from any @kotm-game.com addresses. Once a reply is received, the application will be looked over and will be judged for approval. If you send in an incomplete, or inaccurate application, a letter will be sent to you after verification with the data that needs to be corrected.
